The 12-week assessment model
Holt House provides a focused 12-week assessment model of care, giving the person time, consistency and specialist input while we develop a detailed understanding of their needs, strengths, risks, communication and the support that will help them thrive. We recognise that 12 weeks is a short period when someone has complex needs and the right long-term placement may take time to identify. Our commitment is to continue supporting the individual beyond the initial assessment period until the right placement is secured and the transition is completed safely. Once a suitable placement has been identified, we ask for four weeks’ notice to plan and deliver a thoughtful, well-supported transition.
We create a calm, predictable environment and begin building trust. The team gathers information from the person, family and professionals to understand what matters, what helps and what may be contributing to distress.
Through day-to-day support and specialist observation, we identify practical strategies, communication approaches and positive behaviour support that can make a meaningful difference.
We bring learning together with the wider network to develop clear recommendations and a realistic transition plan, focused on the right long-term setting and support.

Working in partnership
We work in partnership with the person, their family or advocates, commissioners, social workers, health professionals and future providers. Regular communication and shared planning help everyone understand the assessment, contribute their expertise and stay focused on the person’s goals.
The person’s voice, preferences, communication and aspirations guide every stage of assessment and planning.
We bring together the relevant people around the person so decisions are informed, timely and practical.
We work with the receiving setting and wider network to make sure learning is carried forward into the next stage of support.
Multidisciplinary working
Our assessment is strengthened by multidisciplinary working. We coordinate relevant professional input alongside day-to-day observations, so recommendations reflect the whole person rather than a single moment or perspective.
Understanding the function of distress, identifying proactive strategies and shaping practical support plans.
Contributing relevant health, mental health, medication and therapeutic insight to the assessment.
Helping align assessment findings, funding, care planning and the right long-term pathway.
Sharing lived knowledge and helping ensure the next setting understands what matters to the person.
Clear reporting
We provide clear, person-centred reporting throughout the placement. Reports bring together observations, specialist input, progress, risks, strengths and practical recommendations, giving commissioners and the wider network a shared evidence base for next-step planning.
